Only recently saw your mail. As a partial answer to your query, there are two authors who are studing and have written on the religious life of the Appalachains. Their books will be helpful..First,Howard Dorgan, Prof. at Appalachain State..several booka but "In the Hands of a Happy God" is great, and his others are also good. Second, Elder John Sparks' book, "The Roots of Appalachian Christianity" is a good account of the famous Shubal Stearns, Daniel Marshall the "Separate" Baptists and the Regulators in Appalachia.
