Since the GRO website is asking you for a district name *and* number, I'm guessing you're trying to order a certificate for a death which occurred in or after 1993?

That being the case, the district number you need is what Ancestry calls the District and Subdistrict, and for Wandsworth it will start with 256 and be followed by another number and a letter.

From April 1969, the maiden name of a married woman should be included on the death certificate, if the informant knew it, as pointed out already. :-)
